RESUMO

In order to expand the breadth and depth of pharmaceutical services, in March 2022, a tertiary hospital opened a physician-pharmacist joint clinic based on clinical specialty clinics. The hospital formulated a fixed outpatient scheduling system, clarified service targets, established outpatient treatment processes and quality management systems, and standardized pharmacist communication models, to provide patients with " one-stop" standardized pharmaceutical services. As of December 2022, the pharmaceutical joint outpatient service had opened more than 100 consultations and served 1 709 patients. This practice provided reference for promoting the high-quality development of pharmaceutical services in medical institutions in China.

RESUMO

PURPOSE: The aim of this study was to compare the efficacy of surgery combined with conventional chemoradiotherapy in the treatment of limited-stage small cell lung cancer (LS-SCLC), and analyze the factors affecting prognosis. METHODS: A total of 122 LS-SCLC patients were diagnosed via histopathology, of which 61 were operated combined with chemoradiotherapy (comprehensive treatment group), and 61 underwent conventional chemoradiotherapy (chemoradiotherapy group). The Kaplan-Meier method wand log-rank test were used to analyze the overall survival of the patients. Cox proportional hazard regression model was utilized for multivariate analysis of prognosis. RESULTS: The median survival time of the patients was 27 months in the comprehensive treatment group and 22 months in chemoradiotherapy group. The 1-, 3- and 5-year survival rates were 91.8% (56/61), 49.2% (30/61) and 31.1% (19/61), respectively, in the comprehensive treatment group, and 80.3% (49/61), 32.8% (20/61) and 23.0% (14/61), respectively, in the chemoradiotherapy group. The results of log-rank test on the overall survival rate of the two groups of patients revealed that the overall survival rate was overtly higher in the comprehensive treatment group than that in the chemoradiotherapy group. According to stratification analysis of the TNM stage, the 1-, 3- and 5-year survival rates of the patients with stage I + II LS-SCLC were evidently higher in the comprehensive treatment group than those in the chemoradiotherapy group. Multivariate analysis results uncovered that the clinical TNM stage was an independent factor affecting the survival time of the patients. CONCLUSIONS: Surgery combined with chemoradiotherapy may benefit the patients with stage I and II LS-SCLC, while radiotherapy combined with chemotherapy is more suitable for the patients at stage III. TNM stage is an independent factor affecting the prognosis of LS-SCLC.

Based on the GAPS(goal, analyze, problem, solution) management model, the authors analyzed the problems existing in the practice of prescription audit and the strategies of continuous optimization. Multi-disciplinary team(MDT) and evidence-based practice were applied to help the continuous optimization of prescription audit practice and promote the rational drug use management of medical institutions. Through the establishment of accurate control process, the personalized management of prescription dosage of chronic diseases, perioperative drugs, special grade antibiotics and auxiliary drugs was realized. The practice of prescription audit based on the GAPS management model, on the one hand, could improve the quality and efficiency of prescription audit, gradually improve the qualified rate of prescription, strengthen clinical use intervention and promote rational drug use; On the other hand, it could reflect the value of pharmacists′ professional technicians, provide patients with more high-quality pharmaceutical care, and gradually realize the prescription audit mode of " improving quality, controlling cost and increasing efficiency" .

Objective:To evaluate the effect of permissive high stroke volume variability (SVV) on postoperative delirium (POD) in elderly patients undergoing laparoscopic hepatectomy.Methods:A total of 100 elderly patients of either sex, with body mass index of 18.5-24.9 kg/m 2, aged 65-80 yr, of American Society of Anesthesiologists physical status Ⅱ or Ⅲ, with Child-Pugh grade A or B, scheduled for elective laparoscopic hepatectomy, were divided into 2 groups ( n=50 each) using a random number table method: permissive high SVV group (group S) and control group (group C). Group S was given goal-directed fluid infusion with a target of high SVV (13%-20%), and fluid replacement was performed according to the intraoperative CVP in group C. Oxygen extraction ratio was calculated on admission to the operating room (T 0), at skin incision (T 1), at the beginning of liver resection (T 2), completion of liver resection (T 3) and at the end of operation (T 4). Intraoperative blood loss, volume of fluid infused and urine volume were recorded.Lactic acid concentrations were measured at T 0 and T 4.Regional cerebral oxygen saturation (rSO 2) was continuously recorded, and the maximum percentage of decrease in rSO 2 from baseline (rSO 2max%) was calculated.Peripheral venous blood samples were collected at 1 day before operation, at the end of operation, and at 1 and 3 days after operation to determine the concentrations of serum S100β and neuron-specific enolase.POD was evaluated at 1, 3 and 7 days after operation, and the hospitalization time was recorded. Results:Compared with group C, the intraoperative blood loss was significantly decreased, the volume of fluid infused and urine volume were increased, the lactic acid concentration at T 4, rSO 2max%, oxygen extraction ratio at T 2-4, serum S100β and neuron-specific enolase concentrations and incidence of POD at 1 and 3 days after operation were decreased, and the hospitalization time was shortened in group S ( P<0.05). Conclusion:Permissive high SVV can decrease the development of POD in elderly patients undergoing laparoscopic hepatectomy, which is related to reduction of cerebral oxygen metabolism.

After the outbreak of COVID-19, due to environmental pollution in the isolated ward and operational constraints caused by protective clothing and other factors, intravenous drugs in the designated hospitals should be centralized. Combined with the existing process mode, the process of pharmacy intravenous admixture services was optimized, the operation mode of the operation platform was adjusted, the scheduling mechanism was optimized, and the node control process of " first verify and five checks" was explored and developed, so as to meet the 24 h needs of COVID-19 patients. According to the characteristics of COVID-19 drug treatment, the focus of prescription review was adjusted to ensure the drug safety of patients. Other measures included implementing paperless prescription to reduce unnecessary media; implementing segmented infusion distribution management to ensure no cross infection; hierarchical control and configuration environment, strengthening the protection and management of pharmacists, to avoid the risk of personnel infection; optimizing human resource allocation and improving work efficiency. This process reengineering and optimization established the emergency management mode of centralized intravenous drug deployment under the background of COVID-19, which ensured the intravenous drug demand and safety of COVID-19 patients. The treatment work was carried out orderly, and could provide reference for the pharmaceutical department in medical institutions to deal with major public health emergencies in the future.

In designated hospitals for critical patients with COVID-19 in Wuhan, time-efficient pharmaceutical emergency protection system was of great significance for epidemic prevention. Described in the paper are measures taken by the pharmaceutical department of the hospital as follows. These measures include launching an emergency response mechanism, formulating a catalogue of COVID-19 key therapeutic drugs, urgently purchasing therapeutic drugs, transforming the processes of emergency pharmacy, establishing a drug donation management system, building a COVID-19 pharmaceutical care team, and setting up a " cloud pharmacy" to meet the drug needs of patients with non-COVID-19 chronic diseases, in addition to strengthening personnel protection of pharmacists. During such an epidemic, the pharmacy administration works in a professional, comprehensive, complex and systematic emergency program, which guaranteed the safety of drug supply, medication and enabled the treatment to be carried out in an orderly manner.

Objective@#To study the molecular epidemiology and genetic variation of coxsackievirus B5 (CV-B5) in certain areas in China.@*Methods@#MEGA 6.0 software was used to analyze the complete VP1 region of CV-B5 isolated strains from certain areas of China by retrieving the GenBank nucleotide database. Besides, the phylogenetic tree was constructed, the homology of nucleotide and amino acids were calculated and the rate of evolution was estimated.@*Results@#A total of 189 Chinese CV-B5 isolated strains were included in this study. Most of Chinese CV-B5 isolated strains belonged to genotype C, accounted for 90.5%. Compared with the genotype A, the homology of nucleotide sequences and amino acid sequences of complete VP1 region of 189 Chinese isolated strains were 79.8%-82.8% and 92.6%-97.9%, respectively; moreover, the nucleotide and the amino acid homology of 189 Chinese CV-B5 isolated strains among themselves ranged from 80.3% to 100.0% and ranged from 91.5% to 100.0%, respectively. The estimated rate of evolution of the CV-B5 was 4×10-3 substitutions/site/year.@*Conclusions@#The majority of CV-B5 isolated strains belonged to genotype C, and subgenotype C1 and C2 were co-circulating together in certain areas of China.

Objective:To investigate the pharmacokinetics of erlotinib in SD rats with undernourished status. Methods:The ani-mal model of undernourished status was established. The rats were randomly divided into the normal control group and the undernour-ished model group. The different groups were administered with erlotinib(6.25 mg·kg-1) via tail vein,and administered with erlo-tinib (13.5 mg·kg-1) oral solution or oral suspension. The plasma concentration of erlotinib was determined by HPLC. The pharma-cokinetic parameters were calculated. Results:Compared with that of the normal control group, Tmaxin the rats with undernourished status was significantly delayed(P<0.05),while AUC0~tand AUC0~∞were without notable changes(P>0.05). As for the oral sus-pension,Tmaxwas shorter,and AUC0~tand AUC0~∞were higher in the rats with undernourished status than those in the rats with nor-mal status(P<0.05). CL in the rats with undernourished status treated with oral solution or oral suspension was 10% lower than that in the rats with normal status(P<0.05). Conclusion:Undernourished status has a significant impact on the pharmacokinetics of er-lotinib in vivo. It is suggested that the treatment regimen of erlotinib should be adjusted for the patients with undernourished status.

Objective To explore the effect of family nursing mode centering on the timing theory on the rehabilitation effect of patients with acute myocardial infarction and the ability of family care and quality of life. Methods 113 patients with acute myocardial infarction and their caregivers and in accordance with the inclusion criteria in our hospital from January 2015 to January 2017 were selected as the research object,through a randomized controlled trial,they were randomly divided into two groups,of which 56 patients received only routine clinical department of cardiology nursing,and regular followed up patients,included in the control group;the other 57 patients in the routine nursing,the timing theory,carried out family nursing mode of science,system,included in the study group.The recovery of the two groups of patients after treatment was evaluated,and the family members' care ability and quality of life of the two groups were observed. Results Before intervention,there were no statistically significant differences between two groups of caregivers, the intervention study group of caregivers care ability scale (FCTI), caregiver stress scale(CSI) and the perception of the disease scores were (4.82 ± 0.53),(4.72 ± 0.63), (85.93 ± 5.42),the control group were(12.68 ± 1.40),(7.15 ± 0.84),(70.69 ± 5.72), there was significant difference between two groups(t=39.599, 17.417, 14.540,P<0.05). After the intervention,the blood sugar,blood pressure,blood lipid and other basic diseases related indicators in the study group were better than the control group,the difference between the two groups was statistically significant, P<0.05. The quality of life of two groups was found, life quality score for living conditions of the patients in the study group were higher than those in the control group,and the quality of life evaluation was higher,the difference was statistically significant(t=4.320-7.842,P<0.05). Conclusion The application of high value family nursing model to the time centered theory in the treatment of acute myocardial infarction patients and their families,to meet the needs,significantly improve the patient's ability to care for the family,for the promotion of the patients,the clinical significance of improving the quality of life is of vital.

<p><b>OBJECTIVE</b>To investigate the clinicpathologic features and diagnosis of plasmablastic lymphoma (PBL).</p><p><b>METHODS</b>Eleven cases of PBL were collected and followed up, with review of the literature. HIV and EBV status and their relationships with the tumor were specially compared as well.</p><p><b>RESULTS</b>In the current cohort, 10 patients were serologically HIV negative; the male to female ratio was 8 to 3, and the median age was 57 years. Ten cases showed extranodal involvement and one case was nodal based. At presentation, five patients had mid-facial involvement, including sinonasal area (3 cases) and oral cavity (2 cases). Histologically, six were PBL of oral mucosa type, and five were PBL with plasmacytic differentiation. In all cases, the neoplastic cells expressed CD138 and MUM-1, and were negative for CD20 and CD3ε; the median Ki-67 index was 80%. Five cases were EBER1/2 in situ hybridization positive. IgH or/and Igκ gene rearrangement was detected in all five cases examined.</p><p><b>CONCLUSIONS</b>Most patients were no congenital or acquired immunodeficiency in the retrospective study. Of the died patients, EBER1/2 in situ hybridization were negative and their disease staging were Ⅳ, The neoplastic cells were immunoblastic or plasmablastic, sometimes the plasmacytoid cell can be seen and the neoplastic cell had mature plasma cell phenotype, the pathologic diagnosis of the lymphoma is still controversial now. Differentiate with plasma cell neoplasm is difficult, it is necessary to accumulate more cases for advanced study and observation in the future.</p>

Objective:To evaluate therapeutic efficacy and adverse reactions of synchronous chemoradiotherapy combined with gamma knife therapy for pelvic lymph node metastasis of cervical cancers. Methods:Data of 42 cervical cancer patients who suffered from residual pelvic lymph node metastasis and received concurrent chemoradiotherapy were retrospectively analyzed. Intensity-modu-lated radiotherapy was used in the treatment. The prescribed doses of planning target volume and pelvic metastasized lymph node of the planned gross tumor volume were 50.4 Gy/28 F and 59.92 Gy/28 F, respectively. The combined internal irradiation dose was 6 Gy/6 F. Concurrent chemotherapy was administered with 40 mg/m2·w cisplatin. Three months after chemoradiotherapy was completed, the pa-tients with residual pelvic positive lymph node received additional dose ranging from 10 Gy to 15 Gy at three or four fractions by using a gamma knife. Results:Near-term efficacy was 83.3%(35/42) in three months. Local control rates were 88.1%(37/42), 83.3%(35/42), and 76.2%(32/42) in 6, 9, and 12 months, respectively. The 1-and 2-year survival rates were 77.5%(31/40) and 70.0%(28/40), re-spectively. The incidence rates of radiation enteritis, proctitis, cystitis, gut toxicity, and neutrocytopenia were 11.9%(5/42), 38.1%(16/42), 7.1%(3/42), 90.5%(38/42), and 85.7%(36/42), respectively, and the majority of these conditions were classified as grades I and II. Conclusion:Synchronous chemoradiotherapy combined with gamma knife therapy is an effective and feasible treatment method for pelvic lymph node metastasis of cervical cancer;this method exhibits a minimal adverse reaction.

Objective: To evaluate the clinical effect and adverse reactions of radioactive seed interstitial brachytherapy com-bined with etoposide (EP) regimen concurrent chemoradiotherapy treatment for locally advanced non-small cell lung cancer. Methods:All 24 cases of locally advanced non-small cell lung cancer received three-dimensional conformal intensity modulated radiation therapy by using 6 MV X-ray to obtain 95%60-66 Gy/30-33 F planning target volume. All cases received radiation therapy five times a week. EP regimen chemotherapy concurrent with radiotherapy was given to 24 patients. The therapy included VP-16, 60 mg/m2 intravenous in-fusion for 1 d to 5 d, and DDP 50 mg/m2 intravenous infusion on the 1st, 8th, and 28th therapy day. Chemotherapy with EP regimen was given for four cycles, in which two cycles were given during radiotherapy and the remaining cycles were completed after radiotherapy. The patients were reexamined three months after concurrent chemoradiotherapy was completed. Patients with residual tumor, con-firmed via positron emission tomography/computed tomography, underwent 125I radioactive seed implantation interstitial brachytherapy to complement the dose of tumor. Results: The response rate was 83.3% (20/24); the local control rates of 3, 6, 9, 12, 18, and 24 months were 87.5% (21/24), 83.3% (20/24), 75.0% (18/24), 70.8% (17/24), 58.3% (14/24), and 50.0% (12/24). The median survival was 20.2 months. The one-year survival rate was 62.5%, and the two-year survival rate was 37.5%. The following main toxicities were observed:the incidence of radiation-induced lung injury was 25%;the incidence of radiation esophagitis was 33.3%;the incidence of grades Ⅰ to Ⅱ gastrointestinal reactions was 82.3%; the incidence of reducing neutropenia was 87.5%, in which the incidence of gradesⅠtoⅡwas 75.0%, gradeⅢwas 12.5%, and gradeⅣwas 0%. Conclusion:EP regimen concurrent radiotherapy and chemo-therapy combined with radioactive seed interstitial brachytherapy for locally advanced non-small cell lung cancer is effective and has few serious adverse reactions, thus making this approach worthy of promotion.

OBJECTIVE@#To analyze the status quo of quantitative classification in Hunan Province catering industry, and to discuss the countermeasures in-depth.@*METHODS@#According to relevant laws and regulations, and after referring to Daily supervision and quantitative scoring sheet and consulting experts, a checklist of key supervision indicators was made. The implementation of quantitative classification in 10 cities in Hunan Province was studied, and the status quo was analyzed.@*RESULTS@#All the 390 catering units implemented quantitative classified management. The larger the catering enterprise, the higher level of quantitative classification. In addition to cafeterias, the smaller the catering units, the higher point of deduction, and snack bars and beverage stores were the highest. For those quantified and classified as C and D, the point of deduction was higher in the procurement and storage of raw materials, operation processing and other aspects.@*CONCLUSION@#The quantitative classification of Hunan Province has relatively wide coverage. There are hidden risks in food security in small catering units, snack bars, and beverage stores. The food hygienic condition of Hunan Province needs to be improved.

Objective To investigate the effect of breviscapine on lung injury in children undergoing open heart surgery with cardiopulmonary bypass(CPB).Methods Forty-five ASA Ⅱ or Ⅲ children aged 3-65 months weighing 5-21 kg undergoing open heart surgery with CPB were randomly assigned to 3 groups ( n =15 each):control group (group C),low dose breviscapine group (group B1 ) and high dose breviscapine group (group B2).Normal saline 15 ml(group C),breviscapine 0.5 mg/kg (group B1 )or 1.0 mg/kg(group B2 ) were injected iv over 30min after anesthesia induction.Blood samples were taken before operation ( T0 ),at 30 min and 1 h of aortic unclamping (T1,T2 ),at 3 h and 6 h after operation (T3,T4 ) for determination of plasma procalcitonin (PCT)and neutrophil elastase(NE) concentrations.PaO2 and PaCO2 were recorded at T0,T3,T4 for caculation of oxygenation index (OI) and alveolo-arterial oxygen partial pressure difference (PA-a O2 ).Results There were no significant differences in OI and PA-a O2 among the 3 groups( P > 0.05).Plasma concentration of PCT was higher at T1~4in 3 groups,and plasma concentration of NE was higher at T1 in group C than that at T0 ( P < 0.01 ).Plasma concentrations of PCT and NE were lower in groups B1 and B2 than in group C ( P < 0.01).There were no significant differences in plasma concentrations of PCT and NE between groups B1 and B2 ( P > 0.05).Conclusion Breviscapine(0.5,1 mg/kg) can inhibite systemic inflammatory response and attenuate lung injury in children undergoing open heart surgery with CPB.

RESUMO

The theory of four properties (Qi) and five tastes (Wei) is the core of the property theory of Chinese materia medica. It is known that Qi and Wei are associated with the pharmacological effects (Xiao) of herbs. This study took records of all 365 Chinese herbs in Shennong's Classic of Materia Medica (Shennong Ben Cao Jing) as the data resource and established a three-dimensional data cube, in the purpose of finding out and analyzing the frequent patterns and valued association rules of Qi, Wei and Xiao based on Apriori algorithm. The results of this study may give rise to innovative ideas and methods in research of traditional Chinese materia medica.

<p><b>OBJECTIVE</b>To detect the expression of cytokines by acute promyelocytic leukemia (APL) cells before and after exposure to arsenic trioxide.</p><p><b>METHODS</b>Diagnoses were performed according to the FAB cytological classification criteria and cytogenetic criteria. Bone marrow or blood samples from APL patients were collected in heparinized tubes, then primary APL cells were separated by traditional Ficoll-Hypaque density centrifugation and purified after adherence to plastic surfaces. IL-1(beta), IL-6, IL-8, TNF alpha and G-CSF levels in the leukemia cell culture supernatants were detected by ELISA. At the same time, nitro blue tetrazolium (NBT) reduction test was used to detect the differentiation of APL cells.</p><p><b>RESULTS</b>After 96 hours exposure to arsenic trioxide, 10 - 6 mol/L in vitro or 10 mg/d in vivo, APL cells showed a significant increase of IL-1(beta) (P < 0.05) and G-CSF (P < 0.05) production, and a significant decrease of IL-6 (P < 0.05) and IL-8 (P < 0.05). However, there was no obvious variation of TNF alpha when compared with APL cells without exposure to arsenic trioxide. On the other hand, the proliferation ratio of APL cells in vitro was statistically correlated to the IL-1(beta) secretion ratio or G-CSF secretion ratio. The cell number ratio in patients with detectable IL-1(beta) or G-CSF was higher than that without detectable IL-1(beta) or G-CSF.</p><p><b>CONCLUSION</b>IL-1(beta) and G-CSF secretion may play an important role in the proliferation of APL cells after exposure to arsenic trioxide.</p>

RESUMO

<p><b>OBJECTIVE</b>To determine whether autoantibodies against beta(1)-adrenergic and M(2)-muscarinic receptors are related to patients with congestive heart failure (CHF).</p><p><b>METHODS</b>Both synthetic peptides corresponding to amino acids sequence 197-222 and 169-173 of the second extracellular loops of the beta(1) and M(2) receptors were used as antigens to screen sera from 265 patients.188 were congestive heart failure ( CHF) patients with different heart diseases, among them 42 were ischemic cardiomyopathy (ICD) and 52 were idiopathic dilated cardiomyopathy (IDCM) 44 were hypertensive heart disease (HHD) 50 were rheumatic valvular heart disease (RVHD); 77 were controls, among them 36 were simple hypertension and 41 were healthy donors (NC).</p><p><b>RESULTS</b>Positive sera for beta(1)-adrenergic receptor was found in 45.73% (86/188) of CHF patients, while in the controls it was 10.4% (8/77) (P < 0.01); positive sera for M(2)-muscarinic receptor in CHF patients was found in 49.5% (99/188), while in the control it was 11.7% (9/77) (P < 0.01). The positive ratio of autoantibodies against beta(1)-adrenergic and M(2)-muscarinic receptors in CHF patients with cardiac function class II-III (NYHA) were significantly higher than cardiac function class IV. The average titer of autoantibodies against beta(1)-adrenergic and M(2)-muscarinic receptors of the former was significantly higher than the latter; 56.1% of patients with autoantibodies against beta(1)-adrenergic receptor had autoantibodies against M(2)-muscarinic receptor.</p><p><b>CONCLUSIONS</b>Autoantibodies against beta(1)-adrenergic receptor and M(2)-muscarinic receptor were found in sera from heart failure patients with different cardiac diseases. We propose that autoantibodies against beta(1) and M(2) receptors are not only related to the IDCM, but also to cardiac structural and functional changes.</p>

AIM: To examine the electrophysiological characteristics of transient outward potassium current(Ito 1) in repolarization 1 phase from the canine right ventricular M cells. METHODS: By use of whole cell patch-clamp technique, we quantitatively researched the ionic intensity, density of Ito 1 and the notch magnitude of action potential in repolarization 1 phase. RESULTS: (1) The activating process of Ito 1 of canine right ventricular M cells presented evident voltage-dependency.
